Incident: Create Request not making a Request

After our most recent update we have encountered an issue where using the Create Request in the Incident context menu (screenshot below) does not actually create a request. We are looking into the fix for this issue and will be updating this article as progress has been made.
In the interim period, the current workaround is to create a new request with the appropriate item and attach the Incident as a parent in the RITM ticket.
