Lost Paper Paycheck


I received a paper check from Payroll Services and now it's been lost. How do I request a replacement payroll check?

 

A replacement payroll check can be requested one week after payday by contacting Payroll Services via phone, email or in person. The following information must be provided by the employee or departmental contact in order for Payroll Services to research the lost paycheck:

After Payroll has confirmed the paycheck status, a request form must be completed and signed by the employee. The form is available from Payroll Services and can be completed in the Payroll Services Office, Main Building (MAI), Room 4 or emailed to the employee. Completed forms must be submitted to Payroll Services in person or via email.

Replacement checks are generally available within two working days after Payroll Services receives the signed form, and the employee is notified when the replacement check is ready for pick up.

Note: Payroll Services encourages employees to use direct deposit to eliminate the possibility of receiving late payment due to a lost check.
